UK FASHIoN, a 348-patient pragmatic multicentre RCT, found hip arthroscopy improved hip-related quality of life more than a personalised physiotherapist-led programme at 12 months (adjusted mean iHOT-33 difference 6.8, 95% CI 1.7-12.0; p=0.0093), just exceeding the 6.1-point minimum clinically important difference. Both groups improved substantially, and five of six serious adverse events in the surgical arm were treatment-related.

Bacterial septic arthritis of a native joint is a medical emergency with high morbidity and mortality; diagnosis is largely clinical and depends on timely diagnostic aspiration of the joint plus appropriate antibiotics. A hot, swollen, acutely painful joint - especially with fever or immunosuppression - is an emergency-department problem, not a clinic-appointment problem.

4. Among 179 patients (mean age 82) presenting with hip pain and NORMAL plain radiographs, further imaging revealed an occult hip fracture in 71 and a pelvic or acetabular fracture in 34. The authors advocate a high index of suspicion and early imaging for older patients with non-specific hip pain after a fall - a normal x-ray does not rule out a fracture.

Pooled analysis of national joint registries (Australia and Finland, 215,676 hip replacements) put 25-year all-cause construct survival at 57.9% (95% CI 57.1-58.7); pooled case series gave 77.6% (95% CI 76.0-79.2). The authors conclude patients and surgeons can expect a hip replacement to last 25 years in around 58% of patients.

In a three-arm randomized trial of 204 people with MRI-confirmed gluteal tendinopathy, an eight-week physiotherapist-led education and exercise programme produced success on global rating of change in 51/66 participants at 8 weeks versus 38/65 for a single corticosteroid injection and 20/68 for wait-and-see. Education plus exercise beat the injection at 8 weeks (risk difference 19.9%) and still beat it at 52 weeks (20.4%).
